Additional Requirements in case of Passenger Vessel Claim

  • Sworn statement/Marine protest of the master of the vessel
  • MARINA Certificate of Inspection
  • Clearance from Philippine Coastguard (if applicable)

Cellphone Claim Requirements

A. Damage Claim

B. Loss Claim

  2. Police Report & /or excerpt from the blotter (reported within 24 hours from the time of accident)

  4. Accomplish Claims Report Form within 48 hours.
  5. Any Proof of ownership

IN-LAND MARINE CLAIM

General Requirement for In-land Marine Claim

1. Insurance Policy

5. Assured’s Affidavit of Loss declaring the following:

    • Nature of Business Assured
    • Supplier and Consignee of the cargo involved

    • Total value of the cargo/shipment
    • Estimated damage/loss and salvage values
    • Any other information relative to the loss

6. Non-Waiver Agreement – notarized
7. Photographs of the involved vehicle and cargo

    UPON SETTLEMENT

  • Special Power of Attorney (SPA) in the absence of the Assured/Payee (must be accompanied with two (2) Valid 
Government issued ID’s of both the principal and the representative with photo and signature)
  • Original Duly Notarized Board Resolution or Corporate Secretary’s Certificate if the claim is payable to a corporate entity (must be accompanied with two (2) Valid Government issued ID’s of the Corporate Secretary 
and the Company representative (with photo and signature) *as required by LTO
  • Original Insurance Policy and official receipt (for total loss and carnap claims)

Requirements for Property Claim

General Requirements

  1. Insurance Policy and its Official Receipt
  2. Endorsement / attachments (if any)
  3. Certification from Bureau of Fire Protection
  4. Photographs
  5. Assured’s Affidavit or Statement declaring nature of ownership of the property
  6. Location of property
  7. Extent of damage and salvage value
  8. Value of property damaged or destroyed
  9. Liens and encumbrances
  10. Year the building was constructed and original construction cost (for building loss only)
  11. Any information relative to the loss

Additional Requirements for Building Loss

  1. A Civil Engineer’s or Architect’s detailed estimate (obtain at the expense of the assured) to place the building in the same state as before the fire (No contemplated improvements may be included in the estimate and a proper allowance for age and depreciation, etc. must be allowed).
  2. Certified copy of the building permit
  3. Certified copy of declaration of Real property
  4. Copy of plan of the building or sketch prepared by an Engineer/Architect
  5. Lease contract or agreement if the assured is not the lot owner
  6. Certified lot title
  7. If purchased, submit Deed of Sale

Contents / FFF / Machinery & Equipment Loss

  1. Contract of Lease or Agreement (if Assured is not the building owner)
  2. A complete inventory of all the items contained in the insured premises showing the cost price of each and date acquired.
  3. A detailed inventory or list of the articles damaged or destroyed showing the cost price of each and date acquired.
  4. Records, purchase invoices, delivery receipts or any other proof of acquisition

If insured property is owned by the company or corporation:

  1. Mayor’s Permit/Business License/Certificate of Registration of Business Name
  2. Certified copies of Income Tax Returns for the last two (2) years and accompanying Financial Statements filed with BIR, in case of total loss.

Stock-In-Trade

In Case of Total Loss

  1. Certified copies of Income Tax Returns for the last two (2) years with accompanying Financial Statements filed in BIR
  2. Purchase and sales invoices/Delivery Receipts or vouchers or Official Receipts (Purchase and Sales) / Purchase and sales journal and other business records.
  3. In the absence of invoices, submit supplier’s certificate of itemized sales and deliveries
  4. Latest inventory of merchandise submitted to BIR prior to the loss
  5. Other Pertinent documents relative to the business

In Case of Partial Loss

  1. Certified copies of Income Tax Returns for the last two (2) years with accompanying Financial Statements filed in BIR
  2. Purchase and sales invoices/Delivery Receipts or vouchers or Official Receipts (Purchase and Sales) / Purchase and sales journal and other business records.
  3. Complete inventory or list of all the damage items contained in the Insured premises showing the cost price of each, extent of loss, and salvage value if any.
  4. Other pertinent document relative to the business

Requirements for Travel Insurance Claim

Claim Documents Required:

  1. Fully accomplished claim form
  2. Incident Report/Letter of Request
  3. Copy of Insurance Policy
  4. Original Official Receipt of all payment made
  5. Copy of flight itinerary and boarding pass
  6. Copy of passport (pages with details and dates stamped for entry and departure)

VI. Emergency Return Home Following Death of a Close Family Member

  2. Proof of relationship between Insured Person and the immediate family member
  3. Certification/Affidavit stating the reason for trip curtailment
  4. Original official receipts for the additional fees paid for the return ticket to home country such as no show fees, penalties, etc. with a copy of the new travel itinerary
  5. Other documents that will e required (depending on reason of cutting the trip short)

VII. Baggage Delay

  1. Original Property Irregularity Report (PIR) from airline
  2. Original receipts of articles of basic necessity purchased due to delay of luggage
  3. Original acknowledgement receipt/form stating the exact date and time when the baggage was received by the Assured.

VIII. Compensation for In-Flight Loss of Checked-In Baggage

  1. Original Property Irregularity Report (PIR) from airline
  2. List of contents of luggage with estimated price and date of purchase of each item.
  3. Original certification of settlement of the compensation payment by the carrier.

IX. Loss of Travel Documents

  1. Original Police report from the place where the incident occurred or any document that will show the proof of occurrence
  2. Original receipts of expenses for transportation and/or accommodation to go to the place where documents will be issued

X. Loss of Personal Money

  1. Police Report and any document that will show proof of the possessions such as bank/withdrawal and ATM receipt,etc.

XI. Repatriation of the Immediate Family Member Travelling with the Insured

  1. Original medical report/abstract with Medical History of present illness
  2. Medical certificate from physician or hospital
  3. Clinical test/Laboratory results
  4. Police Report (if applicable Ex: accidents)
